On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] working as a Pick Mod Packer was involved in an incident that occurred near the pick mod. The incident took place while the outfitter was removing a tote from a conveyor. According to initial reports the incident involved her left rear shoulder making contact with a bracket and 2 protruding bolts that attach the conveyor leg to the conveyor line. The outfitter backed into the bracket area and felt a mild contusion. She continued to work and did not report the incident. Later at home the Outfitter discovered a small scratch on her left rear shoulder blade. When she returned to work on [REDACTED] she felt a tight range of motion and mild aching in her left shoulder at approximately [REDACTED]. Response at approximately [REDACTED] included first aid and a call to CorVel. The incident was reported to [REDACTED] then to [REDACTED] at approximately [REDACTED].
