On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] was building a pallet in the shipping department. He turned to grab a box from the line and a crate that he had previously placed on the pallet fell off and struck him on his head. Mr. [REDACTED] notified his supervisor [REDACTED] and proceeded to the First-Aid room to make the CorVel call. CorVel recommended the Outfitter seek treatment. Outfitter was sent to Piedmont Healthcare- [REDACTED] [REDACTED]) He was taken out of work from [REDACTED] thru [REDACTED]. He can return to work with restrictions on [REDACTED] thru [REDACTED]. Doctors notes have been attached.
