operating an electronic pallet jack

On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] working as a Forklift Mechanic was involved in an incident that occurred near the battery room of the maintenance area. The incident took place while the outfitter was operating an electronic pallet jack. According to initial reports the incident involved the Outfitter driving the pallet jack on the way to the battery room when the Outfitter drove over a puddle of water. The water came from plastic totes that had spilled onto the walkway and was not visible to the Outfitter at the time. The pallet jack hydroplaned then jerked once traction was gained. As a result of the incident the outfitter sustained a bruised and swollen right wrist. The pain was not noticed until minutes after the impact at 7 10. Response at approximately [REDACTED] included first aid and a call to CorVel. The incident was reported to [REDACTED] at [REDACTED]. Witnesses included [REDACTED].
