On [REDACTED] I SSM [REDACTED] was contacted by Outfitter [REDACTED] in reference to him injuring his back. Mr. [REDACTED] came by my office after his break to inform me that he was feeling pain in his lower back after lifting ammo. Mr. [REDACTED] previously injured his back but received first aid only on [REDACTED]. Mr. [REDACTED] stated he was on the stock picker he was grabbing ammo and placing it onto the pallet when he felt a sharp pain in his lower back. I notified His supervisor [REDACTED] to meet us at the First Aid room. CorVel was contacted and they recommended First-Aid only. The Outfitter requested to see a doctor and the CorVel nurse gave him directions to Piedmont Medical in [REDACTED]. The Outfitter was instructed to bring his doctors notes before returning to work. Mr. [REDACTED] contacted me by phone and stated that the doctor gave him a prescription and took him out of work for one day.
