Sleeve and Label Machine Assignment

At approximately 915a [REDACTED] was assigned to the Sleeve and Label machine. While working on the East Label machine [REDACTED] noticed a fallen 30lb Cylinder going into the East Sleeve entrance conveyor. In an effort to recover the fallen 30lb cylinder [REDACTED] bypassed the machine guarding by reaching into the entrance of the east sleeve machine which had their left arm up to their shoulder inside of the guarding. When [REDACTED] got their finger on the collar of the fallen cylinder the machine actuated which smashed their left middle finger between an upright 30lbcylinder and the collar of the fallen cylinder that they were trying to pick up. [REDACTED] notified the AGL and GL took [REDACTED] to the 1st aid room for treatment. Due to the nature of the incident [REDACTED] was transported to the ER. [REDACTED] was not wearing gloves during this incident.
