Lifting a box from a rack

On [REDACTED] at approximately [REDACTED] a Wayfair associate working in Bulk picking sustained an injury to the right top to middle back due to lifting a box. The associate states that they were working in aisle [REDACTED] picking when the injury occurred. The associate was hired on [REDACTED] and states that they were pulling a box from inside the racks on [REDACTED]. The associate reported the injury on [REDACTED] at approximately [REDACTED]. The associate reports that their initial pain level at the time of the injury (TOI) was a 10. Upon notification of the injury the associate declined first aid but did ask to speak with the on-call nurse during the call with the nurse the associate stated that they didn’t feel any sharp pain and that they had a tingling sensation in both their elbows and in their legs.
