What happened

On [REDACTED] at approximately [REDACTED] a Wayfair associate working in [REDACTED] sustained an injury to the Right back head and neck area due to a tension load bar falling. The associate states that they were working on outbound trailer TM10639682 at [REDACTED] when the injury occurred. The associate was hired on [REDACTED] and states they were applying two tension load bars on the trailer utilizing the top load bar first and applying the bottom load bar second and as they were tightening the bottom load bar the top load bar gave way and struck the associate on the back right side of their head. The associate reported the injury on [REDACTED] at approximately [REDACTED]. The associate reports that their initial pain level at the time of the injury (TOI) was a #7 10. Upon notification of the injury the associate accepted first aid. During the investigation the site found that the associate was unaware of the risk of applying the top load bar first and applying the bottom load bar and
