On [REDACTED] at approximately [REDACTED] a Wayfair associate working in bulk picking reported a PIV incident due to being struck by another PIV. The associate states that while working in aisle [REDACTED] and while they were picking a couch another associate was driving down on the opposite side and came into contact with the PIV. During the investigation the site found that the incident occurred due to the other associate failing to pay attention to their path of travel.. Details of the items involved :OP 1000. After completing the initial report the associate WAS NOT able to return to their process path. The associate is requesting medical attention. The associate was wearing the proper PPE and was not currently working overtime.
