Descending on a PIV

Around [REDACTED] an associate was performing a task at level 4 of a storage area intending to retrieve an item. Upon reaching the location it was determined that the item was not there; the associate was at an incorrect spot. As the associate descended a pallet was observed protruding from its designated space. The pallet unexpectedly flipped upward making contact with the associates hand. The associates fingers positioned on the armrest at the time of impact were consequently crushed. The associate was on a OP. The associate was not working OT. At the time of injury the associates pain level was a 3. They were treated with an ice pack at [REDACTED] when they reported the incident. After treatment the associate reported a pain level of 5. The associated did contact [REDACTED] as they wanted medical attention.
