On [REDACTED] at approximately [REDACTED] a Wayfair associate reported a pain level of 4 out of 10. While working at receiving dock door 135 the associate fell and injured their back after stepping into a gap between a trailer and the dock. The associate perceived the pain manageable and chose to continue their job task before reporting the incident to their supervisor. ;;After being removed from the work area for self-care the associates pain worsened once they got home prompting them to seek medical treatment at a clinic. The clinic advised the associate to contact their employer regarding the incident. The associate then returned to the facility where a manager contacted CorVel.;;During the investigation it was noted that the associate had been hired on [REDACTED] and had completed Receiving Unloading training on [REDACTED].
