What happened
On [REDACTED] at approximately [REDACTED] an associate attempted to load two couches onto a trailer in dock door 296. While trying to lift freight with PIV the PIV hose burst and spewed hydraulic fluid on the associates face eyes beard and clothes. The associate immediately washed off their face at the eye wash station. The associate complained of feeling nauseated and having an upset stomach as a result of some of the fluid getting into their mouth. [REDACTED] referred the associate to a designated clinic. During the investigation it was identified that the associate was hired on [REDACTED].
