What happened

On [REDACTED] At approximately [REDACTED] while headed to the admin office a Supervisor observed an Associate who works in the bulk department on a Raymond order picker near door 174 holding their shoulder. [REDACTED] asked if they was okay and they responded that they were not. When asked if they were injured the Associate confirmed yes.;;The Supervisor immediately offered medical attention by the emergency services which the Associate declined. followed by asking them to rate their pain on a scale of 1–10 and they reported a 9. The Supervisor kindly had the Associate step off the PIV and accompany them to the wellness room. A witness accompanied them and went to the wellness room together.;;During our conversation the Associate stated the injury occurred at work [REDACTED] possibly pulling a muscle during the process of Order Picking from OP4. They did not report it at the time because they did not believe it was serious and were fearful of losing their job. The Associate mentioned
