Maintenance team members were changing the plates on the 13 refiner for PM2 One team member was holding a plate up to be fastened onto the inside of the assembly While getting the plate in position it started to slip and the team member lost his grip on the plate He attempted to catch the plate under the bottom of it to prevent from falling but was unable to and the plate fell on the tip of his right ring finger from about 1 off the ground
