Before the incident
A maintenance technician midway through their shift was working on a down day (no product being produced) to replace the sprockets on a the line 2 pre-pack metal detector conveyor belt at the infeed end of the conveyor. They had removed the bearing and housing from the control panel side of the shaft and were working to remove the bearing and housing from the non-control panel side. As they removed the housing they lost control of the shaft and it struck their left index finger. This finger was positioned in the small space between the shaft and the conveyor framework (this space is usually occupied by the bearing and its housing) the finger was then caught between the shaft and the framework. The incident resulted in a laceration along top of their left index finger near the edge of the nail between the cuticle and the foremost knuckle of their finger. Another maintenance technician was also working on the line 2 pre-pack metal detector near the outfeed end at the time of this inciden
