Before the incident
Walking quickly down a walkway between two pan conveyor belts on line 1 an experienced production team member slipped on a puddle of shine-spray emulsion and fell impacting their back on the floor. The team member was just under 2 hours into their shift at the time of the incident. The puddle of shine emulsion had spilled onto the floor [REDACTED] minutes prior to this incident. A second team member had started cleaning it up with a floor squeegee immediately after it spilled but abandoned the task to address a pan shortage at the line 1 divider. The IP was on their way to the other side of the same divider to also address the pan shortage when they slipped. The shine system had just been installed and filled. A valve on the sprayer nozzle assembly was left open during reassembly resulting in the emulsion spill when the equipment was turned on.
