Before the incident
Two hours and fifty minutes into their shift a team member recently past their first 90 days with the company was working together with a team member of eighteen months experience to change the bun slicing blade on line 2 packer 5. The newer team member was on the non-operator side of the slicer positioning their end of the band blade onto the de-tensioned mounting wheel. The fingers of their right hand were between the closest point of the wheel and the inside of the guarding framework when the more-experienced team member moved the lever to restore the mounting wheels to their outward full-tension position. As the wheels moved outward the newer operators right ring finger was caught between the wheel and the framework resulting in a fracture of the proximal phalanx finger bone of the right ring finger of the operator.
