Before the incident
Two and a half hours into their shift a Packaging Tech team member with less than 1 year of experience was attempting to unjam a stack of trays on a dolly from the dolly loader of the fresh stacker at the end of production line 1. The dolly was jammed between the dolly ejector and the guiderail framework. The team member passed through the light curtain and attempted to move the dolly free from its jammed position. Stored pneumatic energy was still present in the dolly ejector air cylinder. As the stack and dolly were shifted loose the air present in the dolly ejector cylinder caused the dolly and stack to shoot towards the team member. The dolly and stack rode up the team members feet and struck the team member on their left ankle.
