What happened

An employee was assisting in pushing a cart of long titanium bars (25 feet long) the operator was pushing on the left side of the cart with his left hand on the side of the cart nearest his body and his right hand on right side of the cart near the trailing edge of the cart. As they were moving forward the back right wheel ran up and over the base plate of the guard post causing the cart to tilt towards the operator. As the cart rolled forward and off the base plate the cart tilted back towards the guarding post pinching the tip of the employee’s right pinky finger between the edge of the cart and the guard post.
