Operating his walkie.
What happened
Employee was turning his walkie left with his forks first to drive into a pallet. Employee failed to notice a pot hole and his drive wheel drove over the pot hole causing the steering column of the walkie to jerk hard to the right. This threw the employee from the platform of his walkie. The employee lost contact with his left hand on the handlebar but managed to hold on to the steering column with his right hand. The motion of his body as he was leaving the vehicle caused his right shoulder to twist awkwardly. The employee got back on his walkie drove it about 100 feet but when tried to make a turn he started to feel pain in his right shoulder. The pain then started getting worse. the employee reported the incident to his supervisor. The employee was given an ice pack as well as two OTC pain relievers and Medcore was called. Medcore advised the employee to seek medical attention within 2 hours and recommended Advocate [REDACTED] for treatment. Employee stated he was safe to drive and
