TM stated that while walking through the shipping break room they slipped on water that was on the floor. TM stated the housekeeper had just used a scrubber moments before.

At approx. [REDACTED] TM was walking through the Shipping break room. The housekeeping team had someone cleaning the floors with a scrubber or wet vacuum at this time. TM stated that they slipped and fell due to the floor being wet and is stating some degree of pain or discomfort in right hand right knee right ankle lower back head and neck. [REDACTED] called and TM was advised by [REDACTED] to pursue further medical evaluation at the treatment center that the TM identified as their preferred medical site
