What happened
First test chamber door was stuck in the open position IW was the second employee that stood on side of chamber hammering the door pin to free it up [REDACTED] was standing on the West front side of the test chamber watching IW hammer on door hydraulic cylinder shaft EE2 was on the South side of the acid wash exit end looking North [REDACTED] walked towards the movable control station North of the test chamber front and placed their hands on the controls As IW continued to hammer the door pin [REDACTED] moved the control station towards the front West end of the test chamber IW looked towards [REDACTED] [REDACTED] activated the control station and the test chamber door lowered When the test chamber door lowered the IWs left hand was pinched between the frame and upper door block [REDACTED] immediately raised the test chamber door and the IW removed their hand EE2 ran over to assist IW down IW walked West toward the back break room IW received a laceration to the left pinky finger The l
