Recordable due to Rx EE reports he was setting up a [REDACTED] stand to be able to install a controller on the outside of an AC He had to remove the railing on one side of the stand to have it up against the side of the AC In the process of doing this when he lifted the stand overhead to put it into slots he lost control of the stand and it flipped over striking him on top of his head He did not fall or experience any loss of consciousness at the time He has a tender lump on top of his head The skin is intact This occurred [REDACTED] Then when he was sitting at a computer he started to experience a headache some slight dizziness and he feels like his eyes are very sensitive to the light now Employee when to clinic visit on [REDACTED] diagnosed with head contusion and neck strain given prescription and the following restrictions Kneeling squatting bending stooping pushing pulling twisting 4 hours per day No climbing ladders No driving operating heavy equipment No running Must take presc
