Using a ratchet + cheater bar to remove a stuck bolt within the [REDACTED] Cover extruder head. Multiple previous attempts done in different ways before this attempt. 2 associates were tandem using the ratchet + cheater bar.

Ratchet head slipped causing both associates to fall to the ground. After the fall one associate tried to stand up but could not put any weight on the left ankle and felt pain when attempting and was taken to the ER for a suspected broken foot ankle.
