Attempting to take tube off cylinder at hole press
What happened
On [REDACTED] at approx. [REDACTED] EE on 2nd shift was working their normal duty in tube press his designated workstation. EE had loaded two tubes on the equipment cylinders and engaged the press utilizing the two-hand tie-down. Cycle performed as expected. EE then activated the pneumatic lever to bring the extractor forward assisting the tubes to come off the cylinders. The left tube came off smoothly but the right tube remained stuck on its cylinder. EE states he began to wiggle the tube free with his hand (the extractor was in the rear resting position at this time). EE placed his R index finger in between the tube cylinder and center mount of the assembly. EE alleges the extractor came forward and pinched his R index between the tube cylinder and center mount. He states the extractor was then stuck in the forward position requiring him to reach with his left hand for the control lever on the right side of the station (cross body movement).
