Scraping chipping plastic off of grating on blow-off station
What happened
On [REDACTED] at approx [REDACTED] EE was at Midsize [REDACTED] and began to chip away at plastic grating on the blow-off station using a rounded triangle deburr tool. EE states he had 5 minutes between parts and 'didn 't want to be on his phone ' so instead chose to perform this task. As EE was chipping with his R hand the tool slipped and punctured into his left arm. EE immediately covered wound with R hand and was treated by first responder for initial containment. EE was sent to CentraCare St. [REDACTED] where it was discovered the wound involved the artery of his left forearm. EE underwent surgery at approx. [REDACTED] and continued to be monitored by hospital. Original communication by hospital was to observe EE until [REDACTED]. EHS notified OSHA via federal off-hours line on [REDACTED] at [REDACTED]. [REDACTED] responded on [REDACTED] at [REDACTED]. EHS reiterated incident details and OSHA inspector ([REDACTED] [REDACTED]) said they will be sending a letter for follow up on pre
