Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard in the event a "Home Made" Below-the-Hook lifting device was to fail and/or drop the load: (a)Rod Mill Department - Employees used a "Home Made" Below-the-Hook lifting device to move four (4) thousand pound coils. The device was not rated as to capacity nor labeled and certified as inspected, on or about January 23, 2006. (b)Department number 50 - Employees used a "Home Made" Below-the Hook lifting device to move items weighting in excess of one thousand pounds. The device was a steel alloy hook with a handle welded to it. The device was not rated as to capacity nor labeled and certified as inspected, on or about January 23,2006. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THESE HAZARDS INCLUDES: AS SPECIFIED IN BELOW-THE-HOOK LIFTING DEVICES, ANSI/ASME B30.20-1999, THE RATED LOAD OF THE LIFTING DEVICES SHALL BE LABELED ON THE DEVICE AND CERTIFIED AS INSPECTED" VERIFICATION REQUIRED
