(b)Department number 50 - Employees used a "Home Made" Below-the Hook
lifting device to
move items weighting in excess of one thousand pounds. The device was a
steel alloy hook
with a handle welded to it. The device was not rated as to capacity nor
labeled and certified as
inspected, on or about January 23,2006.

TO CORRECT THESE HAZARDS INCLUDES:
