Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to serious injury and/or death from the uncontrolled release of energy when Cadox 40E mixture was heated beyond its Self Accelerating Decomposition Temperature: a.Building 8-Employees, Dispersion Operators involved with the water removal process of Cadox 40E in the Little "T" (T25 Mixer). Among other methods feasible and acceptable abatement methods to correct this hazard is to install audible and visual alarms to alert operators that acceptable operating temperatures for the Cadox 40E process are being exceeded. In addition fail safe controls shall be installed to automatically shut down the steam to the Cadox 40E vessel jacket when acceptable operating temperatures are being exceeded.res
