Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the conditions listed below:
MUD PUMP STATION-RIG 18: On or about 10-28-11 the employer did not ensure
that
centrifugal pump unit #2 was inspected and maintained.
a) The pump's suction valve was "frozen" shut and could not be opened.
b) The pump was missing a hand lever that is designed to open or close the
suction valve.
c) The pump's exhaust valve was very difficult in opening and closing.
d) A grease zerk on the pump's housing revealed that the pump had not been
greased in a long
period.
Among other methods, one feasible and acceptable abatement method(s) to
correct this hazard
include, but are not limited to, implementing the manufacturer's
recommendations and to follow
American Petroleum Institute (API) RP-54 practices such as in Section
9-13-4, 9.16.1 and/or
the manufacturer.
Pursuant to 29 CFR 1903.19, within ten (10) days of the date of this
citation, the employer must
submit documentation showing that it is in compliance with the standard,
including describing
the steps that it is taking to ensure that the centrifugal pumps are
inspected and maintained.
