Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls and struck by hazards due to an improperly installed auxiliary escape line: a)Ringo Drilling Rig #17: On March 21, 2012 and at times prior there to, employee(s) were exposed falls by not being provided a safe means of alternate escape from the derrick board where the auxiliary emergency line was at too steep of an angle which would result in an employee coming down too fast. b)Ringo Drilling Rig #17: On March 21, 2012 and at times prior there to, employee(s) were exposed falls by not being provided a safe means of alternate escape from the derrick board where the ground line was wrapped around and tied to the derrick mast stand as an anchor point. The derrick mast stand would not withstand the required 3000 pound pull. c)Ringo Drilling Rig #17: On March 21, 2012 and at times prior there to, employee(s) were exposed falls by not being provided a safe means of alternate escape from the derrick board where the "U" bolts on the cable clamps were not on the "dead tail" end of the cable. Among other methods, one feasible and acceptable method of abatement to correct this hazard includes but is not limited to following the manufacturer's installation instructions and the API RP 54-1995 Occupational Safety for Oil and Gas Drilling to ensure that the cable that is to be used for the auxiliary escape line be long enough so that it can be twice the distance of the height of the work platform, ensure that the ground anchor will withstand the required 3000 pound pull and that the cable clamps holding the cable are installed properly.
