a)Truck maintenance shop, on or about July 21, 2011, and at times prior

employer did not ensure employees knew and understood the safe operation

use of hooks with functional safety latches in that the hook in use had a
safety
latch and it was damaged and continued to be used, which exposed employees
to
crushing and struck-by hazards.
Among other methods, feasible and acceptable methods to correct this
hazard include but
are not limited to:
1.Conduct frequent and periodic inspections of the lifting device in
accordance
with
ASME B30.10
