Mike Krueger, conducting work at 474 Parker Avenue, Toledo, OH: On or about August 10, 2011, the employer failed to ensure that a minimum of 3 feet clearance was maintained between the energized power lines (220V) and the ladder jack scaffold that was used by employees while replacing 8/12 pitch roof of the two story residential building. Employees were exposed to electrical shock/burn hazards when erecting, using, and dismantling the scaffold.    No additional abatement information is needed for this item.

29 CFR 1926.503(a)(1):   The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Mike Krueger, conducting work at 474 Parker Avenue, Toledo, OH: On or about August 10, 2011, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while replacing 8/12 pitch roof of the two story residential building. Employees did not use fall protection and were exposed to approximately 21 feet fall hazard from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Mike Krueger, conducting work at 474 Parker Avenue, Toledo, OH: On or about August 10, 2011, the employer did not ensure that an extension ladder, used to access the roof of the two story residential building, was extended at least 3 feet above the roof edge. The employees were exposed to approximately 21 feet fall hazards when using the ladder to access the working level in order to conduct roofing activities.    B.	Mike Krueger, conducting work at 474 Parker Avenue, Toledo, OH: On or about August 10, 2011, the employer did not ensure that an articulated ladder, used to access the roof of the two story residential building, was extended at least 3 feet above the roof edge. The employees were exposed to approximately 9 feet fall hazards when using the ladder to access the working level in order to conduct roofing activities.    Mike Krueger was previously cited for a violation of this occupational safety and health hazard or its equivalent standard, which was contained in OSHA inspection # 311604260, Citation # 1, Item # 2, and was affirmed as final order on July 1, 2008.
