29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.     a) Mike Krueger dba Altogether Roofing LLC, on or about May 6, 2019, at the address of 25368 Fox Hunt Drive, Perrysburg, OH: The employer did not ensure employees were wearing appropriate eye protection while operating pneumatic nail guns to secure asphalt shingles to the roof of the residential structure. Operating employees as well as other employees working in the vicinity were exposed to potential flying object hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet.   a) Mike Krueger dba Altogether Roofing LLC on or about May 6, 2019 at the address of 25368 Fox Hunt Drive, Perrysburg, Ohio: The employer did not ensure the side rails of the 24 feet aluminum extension ladder on the front of the residential home structure was extended at least three feet above the roof. Employees were exposed to falls while using the ladder to access the scaffold and roof.  b) Mike Krueger dba Altogether Roofing LLC on or about May 6, 2019 at the address of 25368 Fox Hunt Drive, Perrysburg, Ohio: The employer did not ensure the side rails of the 24 feet aluminum extension ladder on the side of the garage of the residential structure was extended at least three feet above the roof. Employees were exposed to falls while using the ladder to access the scaffold and roof.  Mike Krueger dba Altogether Roofing LLC was previously cited for a violation of this occupational safety and health standard 1926.1053(b)(1), which was contained in OSHA inspection number 1177422, citation number 2, item number 2c and was affirmed as a final order on March 3, 2017, with respect to a workplace located at 10429 Whiteoak Drive, Perrysburg, 43551.
