29 CFR 1926.20(b)(1): A safety and health program is not initiated and maintained by the employer to provide compliance with the general safety and health provisions of the standard:  (a)    Mike Krueger dba Altogether Roofing: The employer does not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness. This violation was most recently observed on or about September 21, 2017, at the address of 3436 Bayberry Place, Oregon, OH, 43605.

29 CFR 1926.503(a)(1): The employer does not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) Mike Krueger dba Altogether Roofing: The employer does not ensure that employees are trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below. This violation was most recently observed on or about September21, 2017 at the address of 3436 Bayberry Place, Oregon, Ohio, 43605.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system or personal fall arrest systems:  a)  Mike Krueger dba Altogether Roofing: The employer does not ensure employees are protected from falls while performing roofing activities. This violation was most recently observed on or about September 21, 2017, at the address of 3436 Bayberry Place, Oregon, Ohio, 43605, when employees were exposed  to a potential 8 foot fall to the ground below.  To abate this hazard, the employer must ensure that fall protection, such as guardrail systems, safety net systems or personal fall arrest systems, is in place to protect employees from fall hazards when working above six feet in height.
