This violation was most recently observed on August 23, 2017, at the address of 2638 Springwater Dr., Toledo, Ohio where employees atop the structure were exposed to an approximately 19 feet fall. To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls by utilizing one or more fall protection systems.

29 CFR 1926.20(b)(1): The employer does not initiate and maintain accident prevention programs as may be necessary to comply with this part. a) Mike Krueger dba Altogether Roofing: The employer does not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness. This violation was most recently observed on August 23, 2017, at the address of 2638 Springwater Dr., Toledo, Ohio. To abate this violation, the employer must ensure that its employees performing roofing work are provided with programs that assists in identifying and avoiding hazards common to their scope of work. Mike Krueger dba Altogether Roofing was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1177422, citation number 1, item number 1 and was affirmed as a final order on March 3, 2017, with respect to a workplace located at 10429 White Oak Dr. Perrysburg, OH.

29 CFR 1926.503(a)(1): The employer does not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Mike Krueger dba Altogether Roofing: The employer does not ensure that employees have been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below. This violation was most recently observed on August 23, 2017, at the address of 2638 Springwater Dr., Toledo, Ohio where employees exposed to an approximately 19 feet fall were not trained on the appropriate use of fall protection. To abate this violation, the employer must ensure that its employees performing work from elevated surfaces are trained on the methods and means of identifying and avoiding fall hazards.
