29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about June 22, 2022 the employer Mike Krueger dba Altogether Roofing, LLC at jobsite located at 3359 Stonebrooke Ln. Maumee OH, 43537 did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Eight employees using three aluminum extension ladders to access the roof to perform roofing work were exposed to fall hazards 12 feet or greater to the ground below.

Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls and struck by hazards during job site operations, decreasing the probability of employee injury or illness. This violation was most recently observed on or about June 22, 2022 at a jobsite located at 3359 Stonebrooke Ln. Maumee OH, 43537.  To abate this violation, the employer must provide evidence demonstrating that appropriate safety and health programs have been implemented avoid fall hazards on its worksites. Eight employees were exposed to falls from approximately 12 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform residential roofing activities on a single-family home.  This violation was most recently observed on or about June 22, 2022 at a jobsite located at 3359 Stonebrooke Ln. Maumee OH, 43537.  To abate this citation, the employer must demonstrate that portable ladder safety and proper usage training is provided and being used by its employees when accessing roofs and other elevated work surfaces while using portable ladders.
