29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. As of and prior to September 12, 2011, authorized employees performing the following required covered activities did not affix lockout (or tagout) devices to each isolating device: a) during the daily sharpening of the head saw blades by the Sawyer in the sawmill building devices are not affixed to the electrical disconnect panel(s) on the lower level west wall; and b) while making hydraulic line repairs on a nailing machine in the "old pallet shop" devices are not affixed to the air valve used to contol pneumatic energy sources. These tasks require isolation by two devices in the specific procedures. Employees are only provided a single lock and therefore cannot isolate each device when performing these, or similar tasks.

29 CFR 1910.265(c)(4)(iv): Elevated walks. All elevated walks, runways, or platforms, if 4 feet or more from the floorlevel, shall be provided with a standard railing except on loading or unloading sides of platforms. If height exceeds 6 feet, a standard toe board also shall be provided to prevent material from rolling or falling off. As of and prior to September 12, 2011; there were two platforms in the Sawmill with unprotected open sides. Employees are required to access the Sawyer operator booth platform on the open side on a daily basis when accessing the lower level saw blade sharpening platform to sharpen the left and right head saws. The saw blade sharpening platform also has unprotected open sides. During these activities, employees are exposed to a fall hazard that exceeds nine feet.
