29 CFR  1910.332(b)(1): Practices addressed in this standard. Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.  a.  On or about September 18, 2023, employees performing the testing were not trained in safe-related work practices such as, but not limited to, testing energized 480 VAC circuit located in the saw room at the worksite location which resulted burns to an employee's hands and arms.

29 CFR  1910.332(a): Scope. The training requirements contained in this section apply to employees who face a risk of electric shock that is not reduced to a safe level by the electrical installation requirements of 1910.303 through 1910.308.   Note: Employees in occupations listed in Table S-4 face such a risk and are required to be trained. Other employees who also may reasonably be expected to face comparable risk of injury due to electric shock or other electrical hazards must also be trained.  a.  On or about September 18, 2023, employees were not trained in and familiar with the safety related work process required by 1910.303 through 1910.308 in order to reduce the risk of electric shock to a safe level when they attempted to verify whether the 480 VAC circuit(s) electrical panel was energized located in the saw room at the worksite location.

29 CFR  1910.333(c)(2):  "Work on energized equipment." Only qualified persons may work on electric circuit parts or equipment that have not been deenergized under the procedures of paragraph (b) of this section. Such persons shall be capable of working safely on energized circuits and shall be familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.  a.  On or about September 18, 2023,  the employer did not ensure that work on energized electrical equipment was performed only by qualified persons.  Unqualified employees that were not familiar with the hazards associated with work on energized electrical equipment and the precautions necessary to mitigate the hazards attempted to verify whether a 480 VAC circuit(s) electrical panel was energized located in the saw room at the worksite location.

29 CFR  1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  a.  On or about September 18, 2023, the employer did not provide the required injury description on the log on the OSHA Form 300, the OSHA 300 form was lacking the "object/substance" that caused the injury.
