29 CFR  1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a. On or about August 15, 2025, employees were exposed to caught-in hazards at the pop-up cut-off saw line in the old mill location while servicing and performing chain adjustment activities on the Robbins Machine Co. equipment, S/N: 0195-1990, Model Hurricane 20, Pop-Up Cut-Off Saw conveyor line. The employer failed to implement the required energy control procedures, including properly shutting down the machine before performing the servicing task. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employees from hazards associated with the chain and sprocket servicing.
