29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. On or about August 15, 2025, at the old mill location on the pop-up cut-off saw line of the facility, the employer failed to ensure that employees utilized the energy control procedures outlined in the developed Energy Source Evaluation Form when performing the chain adjustment on the Robbins Machine Co., S/N: 0195-1990, Model Hurricane 20, Pop-Up Cut-Off Saw conveyor line. This condition exposed employees to caught-in hazards.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about August 15, 2025, employees were exposed to caught-in hazards at the pop-up cut-off saw line in the old mill location while servicing and performing chain adjustment activities on the Robbins Machine Co. equipment, S/N: 0195-1990, Model Hurricane 20, Pop-Up Cut-Off Saw conveyor line. The employer failed to implement the required energy control procedures, including properly shutting down the machine before performing the servicing task. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employees from hazards associated with the chain and sprocket servicing.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about August 15, 2025, at the old mill location at the pop-up cut-off saw line of the facility, the employer did not ensure that employees were sufficiently trained to recognize the methods and means necessary for energy isolation control when servicing or maintaining the pop-up cut-off saw conveyor line. This includes, but is not limited to, performing the chain adjustment on the Robbins Machine Co. Model Hurricane 20, Serial Number 0195-1990, located on the second floor of the pop-up cut-off saw area, which exposed employees to caught-in hazards.
