29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    (A) Guardrail systems; (B) Safety net systems; (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  a. On or about August 20, 2018, locate in the Old Mill on the log carriage, an employee was involved in the repair of a center arm for a log turner while standing on the equipment's framing without any means of fall protection, thereby exposing the employee to a fall hazard of approximately nine feet.

29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about August 27, 2018, located at in the warehouse at the nailer, an employee was operating a Bostitch nail gun while wearing eye glasses without protective lenses that met ANSI Z87.1-2003, thereby exposing the employee to a struck-by hazard.

Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  a. On or about August 27, 2018, located at in the warehouse at the nailer, an employee was operating a Bostitch nail gun while wearing eye glasses without side protection, thereby exposing the employee to a struck-by hazard.

29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed:  29 CFR 1910.147(f)(1)(ii): Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section;  a. On or about August 20, 2018, located in the Old Mill at the saw carriage area during a log turner stand repair, the employer did not ensure employees were protected from moving parts of a log turner arm while by removing the employees from the equipment when performing testing and positioning activities , thereby exposing the employees to a caught-in hazard.   On August 20, 2018, an employee received a crushing injury to the arm when the log turner was being repositioned during a centering arm repair.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a. On or about August 27, 2018, located in the Pallet Plant at the de-duster, there were two ingoing nip points where the conveyors met the end rollers, exposing employees to a caught-in hazard.

29 CFR 1910.213(r)(4): The mention of specific machines in paragraphs (a) thru (q) and this paragraph (r) of this section, inclusive, is not intended to exclude other woodworking machines from the requirement that suitable guards and exhaust hoods be provided to reduce to a minimum the hazard due to the point of operation of such machines.  a.  On or about August 27, 2018, located in the Old Pallet Plant, employees operated a Whirlwind upcut saw to cut wood material.  The upcut saw did not have suitable guarding to prevent employee exposure to the saw's point of operation, thereby exposing the employees to an amputation hazard.
