In the pallet plant, the employer did not ensure that an authorized employee affixed lockout devices to the electrical and hydraulic energy sources prior to removing the cover (guard) for cleaning inside the Pendu Junior Gang Resaw (model: 2000E; S/N: 8706-AP).  On or about January 29, 2015, an employee was hospitalized after getting their hand caught in the saw blades.     Sawmiller, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 98904, citation number 1, item number 1 and was affirmed as a final order on December 2, 2011, with respect to a workplace located at 17235 Haydenville Road, Haydenville, Ohio, 43127.
