29 CFR 1910.23(a)(2): Every ladderway floor opening or platform guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening:    (a) On or about December 2, 2011 and at times prior thereto, the battery charging area on each end of battery charging platforms A, B, C and D, there were no swinging gates or offsets through the railing to the ladderways.  The platforms were 8 feet high.

29 CFR 1910.212(b): Machines designed for a fixed location were not securely anchored to prevent walking or moving:     (a) On or about December 2, 2011 and at times prior thereto, in the maintenance shop, a Powermate 18 inch drill press, model number 2800, serial number 7040398, was not securely attached to prevent movement as it was not secured to the floor or other platform.

29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15-, 20-, and 30-ampere receptacle outlets that were not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel:    (a) On or about December 2, 2011 and at times prior thereto, in the maintenance shop there was no ground fault circuit interrupter protection on the extension cord in a retractable wheel that hung from the ceiling.  The extension cord powered 110 VAC portable power tools, such as drills and circular saws.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there was entrance to a ramp, stairway, or fixed ladder:    (a) On or about December 2, 2011 and at times prior thereto, the battery charging area on battery charging platforms A, B, C and D, there were openings that varied from about 2 feet to 14 feet wide.  The openings were located in areas where there were no battery charges.  The platforms were about 60 feet long by 4 feet wide by 8 feet high.      AAFES WEST COAST DISTRIBUTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.23(C)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313117533, CITATION NUMBER 2, ITEM NUMBER 1, ISSUED ON JULY 24, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 700 EAST ROTH ROAD, BUILDING 549, FRENCH CAMP, CALIFORNIA.
