29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:  (a) In the maintenance shop on or about May 9, 2012, and at times prior thereto, the work rest was 7/8s inch from the abrasive wheel on a Dayton 6 Variable Speed Bench Grinder.  The wheel was used to grind small metal tools and parts.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:    (a) In the maintenance shop, on or about May 9, 2012, and at times prior thereto, four foot long florescent bulbs were located 78 inches from the floor in a fixture and were not protected from accidental contract.  A DeWalt compound mitre saw was used under the bulbs.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure:  (a) In the maintenance shop, on or about May 9, 2012, and at times prior thereto, a flexible cord was used to power outlets in a 4 inch square box that was mounted to the wall.  The outlets powered a 110 VAC DeWalt compound mitre saw.
