29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a.  On or about August 24, 2012 - at the above addressed jobsite, employees conducting roofing operations were exposed to falls to the inside of the building created by the presence of an unprotected roof hatch.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    a.  On or about August 24, 2012 - at the above addressed jobsite, employees performing roofing operations more than twenty five (25) feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.     THE DURABLE ROOFING COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 109450, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 23, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 9100 S. ST LOUIS, EVERGREEN PARK, ILLINOIS  60805.
