29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: a. On or about June 5, 2014 - at the above addressed jobsite, employees performing roofing operations approximately eighteen (18) feet nine (9) inches above the ground had personal fall arrest systems secured to a ventilation pipe on the roof, the anchor point used would not support at least 5,000 pounds, thereby exposing the employees to fall hazards.

THE DURABLE ROOFING COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(4)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 594978, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 26, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 11020 S. KEATING AVE., OAK LAWN, IL 60453.
