29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a) On or about April 07, 2017, at the above addressed jobsite, employees were performing roofing operations on a roof using pneumatic nail guns and did not wear safety glasses, thereby exposing the employees to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about April 07, 2017, at the above addressed jobsite, employees performing residential roofing operations were not using personal fall protection, thereby exposing the employees to fall hazards of approximately eight (8) feet to nineteen (19) feet,  two (2) inches.    The Durable Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1175164, citation number 1, item number 1 and was affirmed as a final order on November 23, 2016, with respect to a workplace located at 16975 Westview, South Holland, IL.    The Durable Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 594978, citation number 2, item number 1 and was affirmed as a final order on September 26, 2012, with respect to a workplace located at 11020 S. Keating Ave, Oak Lawn, IL.
