29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Olmedo Roofing at 2202 - 69th Ave., Greeley, CO 80634: On or before September 1, 2012, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns. Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph (b). Note: If using these systems was infeasible or created a greater hazard, the employer did not develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502: a) Olmedo Roofing at 2202 - 69th Ave., Greeley, CO 80634: On or before September 1, 2012, the employer did not ensure that employees were using fall protection when shingling this residential roof with a 6 in 12 pitch. Condition exposed employees to fall hazards of up to 15 feet. Olmeda Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 313414112, Citation number 1, Item 2, issued December 7, 2009, and became a Final Order on February 5, 2010, with respect to a workplace located at 2510 White Wing, Johnstown, CO 80534. Olmeda Roofing was previously cited for a repeat violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 107497, Citation number 1, Item 1, issued October 14, 2011, and became a Final Order on December 7, 2011, with respect to a workplace located at 3354 Hewitt St., Loveland, CO 80538.
