29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Olmedo Roofing at 3018 68th Ave, Greeley, CO: On and around July 2, 2020, the exposing and creating employer did not ensure that employees were protected from injury, in that four employees re-roofing a two-story residential home were not utilizing a fall protection system. This condition exposed the employee to a fall hazard of approximately 18 feet. Olmedo Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 1303118, Citation 1 Item 1 issued on April 12, 2018 and became Final Order on May 1, 2018 with respect to a workplace located at 1919 68th Ave, Greeley, CO 80631.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: (a) Olmedo Roofing at 3018 68th Ave, Greeley, CO: On and around July 2, 2020, the exposing and creating employer did not ensure a written fall protection training certification record was prepared, which included the identities of employees trained, dates of training, and the signature of the trainer or employer.
