Michigan St. Sidney, OH: The employer did not ensure employees were protected from head injuries while working below employees conducting construction activities above. Employees were exposed to potential impact injuries caused from falling material and/or equipment.

1926.502(h)(1)(v): The safety monitor had other responsibilities which took the monitor's attention from the monitoring function. a) Technique Roofing Systems, LLC on or about November 19, 2012, at the address of 2115 W. Michigan St. Sidney, OH: The employer failed to ensure the safety monitor's only responsibility was the monitoring function. The designated worker charged with monitoring employee(s) performing roofing work on a low slope roof, partook in activities such as installing roofing material and retrieving material. These actions took his attention away from employee(s) working near the roof's unprotected perimeter. Employees were exposed to a potential 11 ft. fall to the ground below.
