29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about September 1, 2023 the employer Technique Roofing Systems LLC, at jobsite located at 2500 W. State St., Fremont, OH 43420, did not ensure that roof openings where a 3' by 4' roof curb box was removed, were guarded properly to prevent employees from a fall hazard, approximately 15 feet and 3 inches to the floor below. One of three employees performing roofing work activities directly next to the open hole, fell through the open hole 15 feet and 3 inches to the level below suffering injuries resulting in hospitalization. Technique Roofing Systems LLC was previously cited for a violation of an equivalent Occupational Safety and Health Standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 1648504, citation number one, item number one and was affirmed as a final order on March 29, 2023, with respect to a workplace located at 453 Maple St. Elmore, OH 43416. Technique Roofing Systems LLC was previously cited for a violation of an equivalent Occupational Safety and Health Standard CFR 1926.502(f)(1)(i), which was contained in OSHA inspection number 1623179, citation number one, item number one and was affirmed as a final order on November 3, 2022, with respect to a workplace located at 350 Stump Pike Rd., Attica, OH 44807.
