29 CFR 1926.502(h)(2): Mechanical equipment shall not be used or stored in areas where safety monitoring systems are being used to monitor employees engaged in roofing operations on low-slope roofs. a) Verizon Building, an employee operating a mechanized roofing tear off machine alongside the western edge of the flat roof was exposed to an approximate 26 foot fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 11/29/2012. CROWTHER ROOFING AND SHEET METAL OF FLORIDA, INC., WAS PREVIOUSLY CITED FOR VIOLATIONS OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(11) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314492091, CITATION NUMBER 1, ITEM NUMBER 1, WHICH WAS AFFIRMED AS A FINAL ORDER ON 15 DECEMBER 2011 WITH RESPECT TO A WORKPLACE LOCATED AT 2770 REGENCY OAKS BLVD., CLEARWATER, FL.
